Health care insurance has become increasingly complicated. We will bill your visit directly to your insurance provider for all covered services if you supply an updated proof of insurance at each
visit. Please check with us to see if our practice is included in your health insurance network. In some cases a referral from your primary care provider and pre-authorization may be necessary for
services to be covered. Any co-insurance amount is payable at the time of service.
We are happy to file claims for you, but any remaining balance is your responsibility if your insurance company or plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us. Please note that some of the services we provide may not be covered by insurance.

PATIENTS WITH MEDICARE PLEASE NOTE:
We welcome Medicare patients. As a courtesy, we will also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered under Medicare. You are responsible for covering the costs of these services.
