Health care insurance has become increasingly complicated. We will bill your visit directly to your insurance provider for all covered services if you supply an updated proof of insurance at each
visit. Please check with us to see if our practice is included in your health insurance network. In some cases a referral from your primary care provider and pre-authorization may be necessary for
services to be covered. Any copay amount is payable at the time of service.
We are happy to file claims for you, but any remaining balance is your responsibility. Please note that some of the services we provide may not be covered by insurance. You will be
notified of those costs prior to receiving services.
